Greater Augusta HIV/AIDS Community Resource Needs Assessment Survey
This survey is for those that work in the medical field such as employees of healthcare providers, general practitioners, doctors, nurse practitioners, nurses, testing centers, municipal and county health department employees, HIV specialty clinics, and caregivers to those infected or affected by HIV/AIDS.
Purpose of this Needs Assessment:
This is a survey for the Greater Augusta HIV/AIDS community resource needs assessment of our healthcare and service providers.We have money that we are looking to fund efforts in the local community to help prevent and treat HIV/AIDS. This is our first step in the process of obtaining and analyzing information to determine the current status and service needs of the Augusta metropolitan area. Our goal is to gain insight into the current conditions, care, resources, prevention, and approaches that are being used among the Greater Augusta population. The resulting analysis will be used to create a gap analysis for us to prioritize our efforts and facilitate monetary distribution to the greatest needs identified.
This survey will be capturing information such as anonymous population identifiers; HIV status; your current treatment options, etc.
Thank you for taking a few minutes to help us do the most good in our community and direct our efforts in the direction of the greatest needs. We will provide you with our finished data presentation once our project has been completed.
