Opportunity Music Project Application Form

To apply for Opportunity Music Project, please fill out this application form and send your supporting documents via regular mail or email (preferred) to jgarand@opportunitymusicproject.org. Please scan and include your child’s most recent report card (this information is for statistical purposes only).

Students can apply for Opportunity Music Project at any time this year, however all applications will be reviewed on a rolling basis, with a July 15, 2016 deadline for students wishing to enroll for the 2016-2017 academic year.

Upon receiving your completed application, you will be contacted to arrange an interview. All interviews take place at the Opportunity Music Project teaching studio, located at 4 West 43rd Street.

Supporting documents: (1) letter of recommendation, and (2) a short essay about music and what it means to the applicant (this may be written by the parent if the student applicant is younger than eight years old). Please also include a copy of your child’s most recent report card.
