As the parent or legal guardian of the child named above, I hereby give my full consent and approval for my child to participate as a team member in the sport designated below.

I understand that there are certain risks of injury inherent in the practice and play of this sport, as well as in traveling and other related activities incidental to my child’s participation, and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in the designated sport and that my child is healthy and has no physical or mental or conditions that would restrict full participation in these activities, except as listed.

In addition to giving my full consent for my child’s participation, I do hereby waive, release, and hold harmless The Cut Off Youth Center, its officers, coaches, sponsors, supervisors, and representatives for any injury that may be suffered by my child in the normal course of participation in the designated sport and the activities indicated thereto, whether the result of negligence or any other cause.

Designated Sport: Cut Off Youth Center Hurricane Swim Team 

As part of your child’s participation on the Cut Off Youth Center Hurricane Swim Team, your child is expected to behave. For the first offense of issues of pushing, touching others, jumping onto others, destruction of property, and other actions, your child will be given a verbal warning. For the second offense of these already address actions or more serve actions, your child will be asked to sit out the pool for a certain amount of time. If your child’s behavior does not improve then your child will be asked to sit out of the remainder of the practice. If problematic behaviors continue, actions and behaviors will be reported to the director of the Cut Off Youth Center and will be handled at the discretion of the COYC board and director.
