This includes general first aid treatment for any minor injuries or illnesses experienced by my Child. If the injury or illness is life threatening or in need of emergency treatment, I authorize the Designated Adult to summon any and all professional emergency personnel to attend, transport, and treat my Child and to issue consent for any X-ray, anesthetic, blood transfusion, medication, or other medical diagnosis, treatment, or hospital care deemed advisable by, and to be rendered under the general supervision of, any licensed physician, surgeon, dentist, hospital, or other medical professional or institution duly licensed to practice in the state in which such treatment is to occur.
I agree to assume financial responsibility for all expenses of such care. It is understood that this authorization is given in advance of any such medical treatment, but is given to provide authority and power on the part of the Designated Adult in the exercise of his or her best judgment upon the advice of any such medical or emergency personnel.
The following are the steps that will take place when a participant of an Access Sports program uses foul language or is physically aggressive towards another person. The purpose of informing and meeting with families is to quickly address and resolve the situation so that all participants can attend and enjoy future Access Sports programs.
Foul Language/Threatening LanguageIf foul or threatening language is used during an Access Sports program, then the parents/guardians will be notified. If foul or threatening language continues after notifying the parents/guardians, then a meeting will be scheduled with the parents/guardians to discuss the issue. The athlete will not be allowed to attend Access Sports events until this meeting has occurred, and a plan of action is put in place to avoid future incidents.
Physical AggressionIf any form of aggression occurs at an Access Sports program, (i.e. hitting, kicking, pinching, slapping, biting, etc.), the parents/guardians will be notified, and the athlete may be removed depending on the incident. A meeting will be scheduled to discuss this situation and the athlete is not allowed to attend Access Sports events until this meeting has occurred, and a plan of action is put into place to avoid future incidents. If physical aggression continues after the parent meeting, it may be that the athlete will not be allowed to return to that particular program.
Parents and SpectatorsAll parents/guardians and spectators must remain off the field or court once play begins. They shall remain in the stands, bleachers, sidelines, or other such areas which are designated for safety and viewing games. They will not interfere with play. If parents/guardians are needed on the field or court, an Access Sports coach or volunteer will request assistance.
