Love Is Fur Ever Dog Rescue "Adoption Form"
We are so excited that you are ADOPTING!  We know that you will enjoy a LIFE filled with companionship and love.  Thank you for helping us SAVE more dogs by adopting.

The ADOPTION parent(s) have met the requirements for serving as a canine parent from Love is Fur Ever Dog Rescue and is called “ADOPTER/ADOPTIVE parent” in this agreement.
This agreement is between LIFE Rescue and the ADOPTIVE Parent for the purpose of maintaining and continuing the  proper love and care for these dogs in their for ever homes for their LIFE-TIME.

The Adoptive Parent agrees to:
• Provide a loving and safe home for the below-named dog for a life - time from the date below. • Provide shelter, fresh water, wholesome food, adequate exercise, and loving attention to the adopted dog.• Obtain immediate veterinary care if the dog becomes ill or is injured. If the adoptive parent is no longer able to keep the adopted dog (FOR ANY REASON) the Adoptive Parent is required to return the dog to LIFE Dog Rescue OR make arranges to place the dog in another home - This arrangement MUST be communicated and GET APPROVED by LIFE Dog Rescue (Along with a home check and an adoption fee - in which will be obtained by the organization). • When a adopted dog is outdoors - provide the dog with a secure area, including shelter from unsafe elements. • Never tie up or chain the adopted dog at any time. • Never leave the adopted dog in a car for any period of time without the A/C running.  
The adoptive parent is REQUIRED to keep their adopted dog on monthly heartworm prevention, keep them updated on vaccines, and see their vet yearly.  At anytime the rescue is authorized to ask for any and all records to verify proof that the adoptive dog is up to date on all yearly vetting.  If at anytime LIFE Dog Rescue receives information that the adoptive parent/owner is rehoming the dog WITHOUT the organizations consent or knowledge - LIFE Rescue has all rights to claim the dog(s) back under their ownership.  If at anytime LIFE Rescue seeks yearly vet records and they are not provided within 15 business days the organizations has all rights to take the dog(s) back under their ownership.

I understand that this is an adoption, and not a sale, and that dog adoption agencies reserve the right to postpone, refuse, or rescind any adoption. I certify the above information to be true and understand that any false information may result in cancellation of the adoption. I hereby release Love is Fur Ever Dog Rescue, its agents, participating agents, and sponsors from ANY liabilities related to the adoption of this dog. Adoption fee WILL vary for each dog. Signing below gives LIFE Rescue the acknowledgement that you understand the adoption process, you are committed to adopting, and treating this dog as a member of your family. *
