The following form is used for UF's pathology faculty and residents to anonymously express praise or concerns to the administration of the pathology residency program, namely Dr. Saulino, Dr. Middleton, and Jordan.
Praise and concerns should be regarding the program and/or those who occupy it (faculty, residents, program administration, etc.). For example, items submitted on this form could include but are not limited to resident/faculty behavior, clinical performance, residency program policies and/or processes, mistreatment, discrimination, safety, etc.
FOR ABUSE/HARASSMENT:
Depending on the severity of the situation and how comfortable you are with reporting it directly to our department, you are free to report abuse/harassment on...
