After completing the form you will receive an email with a copy of your invoice.

Permit Fees must be made by personal check, company check, cashier's check, or money order.

On the memo line of the check, please include:
• Name of the property owner
• Address of the property for which the permit is being requested
• Or the UPC number if the property address is not available
