As an adult over the age of 18 years, I agree to be involved in the below-described study.
Purpose of the Study: To evaluate the relation between media and prosocial behavior.
Procedures to be followed: Following this consent, I will be asked to answer survey questions designed to evaluate engagement in certain behaviors and attitudes.
Discomforts and Risks: Participation poses minimal risks, those encountered in everyday life.
Benefits: The benefits to me include the chance to learn something about my personality and how it relates to my entertainment choices. The benefits to society include furthering of scientific knowledge on the effect (if any) consuming fiction has on personal development and behavior.
Duration/Time: The survey process will last approximately 30 minutes.
Statement of Confidentiality: I understand no identifying information will be used in any report describing my responses, and only the researcher named above and their professor will be informed of my participation in this activity, unless I ask to have a third party informed for proof of completion. This means I will be randomly assigned an identification number known only to the researcher. Neither my name nor any other type of personally-identifiable information will be requested during the survey. This consent document is the only form bearing my name, and will be stored separately from my data.
Right to Ask Questions: Participants have the right to ask questions and receive appropriate responses to them. In the event I have any questions or concerns about any aspect of this assignment, I will immediately contact the study supervisor, Dr. Tirza Leader, at firstname.lastname@example.org. For questions concerning my rights as a research participant, I will contact the IRB, at email@example.com.
Compensation: Participants may receive satisfaction from knowing they’ve helped contribute to science.
Voluntary Participation and Right to Withdraw: I understand I have the right to revoke this consent at any time, even if ethical standards in research have not been violated. Moreover, even if I choose to continue my participation I may decline to answer some questions. If I refuse to participate there will be no penalty, retribution, or loss of benefits. I understand that, due to the anonymous nature of the survey, once completed and submitted my responses cannot be removed from the final study data.
