GSSP early GSHI activation
This form is to request that the graduate school activate Graduate Student Health Insurance (GSHI) on the RA-TA plan prior to the student's first semester in a graduate degree program. To qualify:
- Must be a brand new graduate student
- Must actively meet all GSSP requirements for the upcoming fall term, including having an active qualifying appointment (approved by all parties) and full-time enrollment
- Must be funded from a non-slot source.

- MUST have US address listed in MyPack Portal.

We will stop accepting requests for Fall 2025 early activation on July 7.
