According to the UK Goverment norms during the COVID 19 Pandemic, all the participants of each services are supposed to provide their personal details and keep them in the Parish Record for 21 days. For each weekend Mass you will have to fill in this form. You can also walk in and get a form to be filled manually at the entrance.
