2025-2026 EHS Attendance Clearance Form
PARENT/ GUARDIAN:
This form can only be used to report the following: 
  • Permit for Early Dismissal
  • Late Arrival
  • Full Day Absence

  • Note Early Dismissal: The attendance office cannot dismiss students for illness or injury; all such cases are handled by the Health Office directly.
    • Students on campus must report to the Health Office in room 320 if they need to leave early due to illness or injury.
    • The Health Office will call the parent/guardian to confirm approval for early dismissal.

  • Note Late Arrival: If your student is arriving late to school, you may fill out the form with the estimated time of arrival. 

    • All students arriving late to class must check into the attendance office before going to their class.

    • If you fill out the form for a specific time and they arrive later you must notify EHS attendance.


For Reasons Other Than Illness:  

  • Parents/guardians must submit a request to the attendance office before the student leaves campus at least 30 minutes before dismissal.

  • Religious Excusals may only be used on Religious holidays. All other Religious based absences go as parent request.

  • The attendance office will issue an off-campus pass, which will be delivered to the student in their classroom.

  • Any student who leaves campus without a permit to leave early pass will be marked truant for any missed classes, so it is essential to follow this procedure to avoid truancy penalties. 

All students arriving late to class must check into the attendance office before going to their class. Students who are 30 minutes + late to class are considered absent and the parent/guardian can verify that absence (Late Arrival) by filling out this form or calling the attendance office at (714) 962-1356 ext. 55428 or 55427. Late arrivals can now be verified with this form.
