I agree for my child to participate in the 2021 Statesboro High Softball Camp to be held July 6-8, 2021 from 9 am - 12 pm. I acknowledge that my child will be participating in softball drills including: hitting, fielding, throwing, running,  and other physical activities under the supervision of the Statesboro High School softball players and coaches. I will not hold the school, softball players, coaches, and/or Mill Creek Park liable or responsible for injuries and/or accidents. I understand that my child will be supervised at all times and will be taught age appropriate, safe, softball instruction. I have also listed any medical conditions and/or allergies which the staff needs to be aware of in order to care for my child. In the event of an emergency, I give the Statesboro High School softball coaches permission to transport my child to a medical facility if necessary. *
