This is the membership application for the Society of Health & Medical Physics Students (SHMPS). If you do not currently have a faculty advisor, please answer "N/A". By submitting this form, you accept all the responsibilities of an organization member- participation (unless you have a prior commitment/emergency) in SHMPS activities to earn membership points. These points are used to determine who is eligible to receive SHMPS funds for conference expenses and/or other expenses associated with professional/academic events.
