housing status this year, in order to help us determine services and support
you may be eligible to receive. This information is also used to ensure
that we receive adequate school funding to support our students and families.
All incoming families for the Fall 2022 semester may have already completed
this form this year and would not need to re-do it.
This form is intended to address the McKinney-Vento Act 42
U.S. C. 11435 and must be completed for each student. McKinney-Vento is
designed to support families who are in a transitional situation at home; It
could be homelessness, living at a shelter, temporarily living with another
family, or any other temporary living situation due to hardship. The
McKinney-Vento Act protects a student’s right to stay in their school
regardless of where they are living, and it ensures that students are provided
with support such as school supplies and a MetroCard.
The information provided in this questionnaire is
confidential. The information is shared primarily with your child’s guidance
counselor, the pupil accounting office, and administrators, but is not shared
with any other educational agency. Your child will not be discriminated against
based upon the information provided.
