In consideration of the Elmhurst Bears awarding this scholarship, I, the undersigned applicant, hereby agree to the following for myself and on behalf of my athlete.

My athlete, and I, will adhere to all safety regulations and guidelines set forth by the Elmhurst Bears organization.

My athlete, and I, will maintain good behavior and conduct ourselves in a manner that reflects positively the values of the Elmhurst Bears, and the scholarship program.

I will volunteer my time and effort to support Elmhurst Bears community initiatives and events when reasonably possible throughout the year.

I acknowledge that any violation of these terms may result in the revocation of the scholarship and/or other disciplinary action deemed appropriate by the Elmhurst Bears organization for the current season and may make me ineligible for future Elmhurst Bears scholarship opportunities.

I understand that failure to return any Elmhurst Bears issued equipment or uniforms (helmet, pads, pants, cheer top, cheer bottoms, footballs, etc.) will make me ineligible for future Elmhurst Bears scholarship opportunities.
