Certificate of School Enrollment Requests
THIS IS FOR JACKSON COUNTY HIGH SCHOOL STUDENTS ONLY!

Please complete this form to request your Certificate of School Enrollment for your Learner's Permit or Driver's License (and ADAP certificate).

All requests are available for pick up the next day after 1:00 pm in the front office at Jackson County High School.  We DO NOT mail or fax Certificate of School Enrollments.

Reminder:  Certificate of School Enrollments are only valid for 30 days once dated.  Please do not submit multiple requests.
