UCC Stewards’ Dance Permission Form 2026–27
We look forward to welcoming Year 11 and 12 students to the Stewards’ Dance on Friday, November 20, 2026, from 8:00–10:30 p.m. in the Student Centre at Upper Canada College. This school-sanctioned event is organized by the Board of Stewards with faculty supervision. This year’s theme is A Great Gatsby Affair; gold and sparkles are encouraged.

Tickets cost $25 per person. Each UCC student may bring one guest from another school, but bringing a guest is entirely optional. A parent or guardian must complete this form by Friday, November 13, and agree to student-account billing: $25 for the UCC student alone, or $50 for the UCC student and one guest.

Any external guest must also complete the separate Guest Permission Form with their own parent or guardian.

The undersigned hereby releases Upper Canada College, its respective directors, officers and employees of and from any and all claims whatsoever arising or which may arise by reason of the student’s participation in any aspect of this event at UCC including any claims due to personal injuries, death or illness. Should the student suffer injury or illness while participating in any aspect of this event at UCC undersigned hereby authorizes any representative of Upper Canada College and, in particular, any staff member accompanying the student to authorize such medical attention for the student as may be deemed appropriate by said representative of UCC in the circumstances.  The undersigned agrees to bear the costs of all medical care and procedures required by the student.  The undersigned also agrees to maintain appropriate medical insurance coverage for the student while at the Steward's Dance. The undersigned hereby releases UCC, its respective directors, officers and employees from any claim arising out of any medical treatment the student may require.The undersigned acknowledges that should the student fail to keep and obey all rules and regulations prescribed by UCC, its respective directors, officers and employees, while participating at this event may, in its sole and absolute discretion, terminate the student’s participation at this event without any refund for the costs.  Any additional costs incurred by reason of the termination of the student’s participation in the trip and/or as a result of the student being sent home will be the responsibility of the undersigned.
