I, the undersigned participant, hereby acknowledge and agree to the following terms and conditions for participating in the All Creature Great and Small 5k:

Assumption of Risks: I understand that participating in the .5k or 5K run involves certain inherent risks and dangers, including but not limited to the risk of personal injury, property damage, and death. I voluntarily assume all such risks associated with my participation in the Event.

Health and Fitness: I attest that I am physically fit and sufficiently trained to participate in the .5k/5k race. I acknowledge that it is my responsibility to consult with a healthcare professional prior to participating in the Event to ensure that I am in good health and capable of completing the .5k/5K run.

Release and Waiver: In consideration of being allowed to participate in the Event, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless the organizers, sponsors, volunteers, and all other individuals and entities associated with the Event from any and all claims, liabilities, actions, or damages that I may have arising out of or in any way connected with my participation in the Event.

Medical Treatment Authorization: In the event of an injury or medical emergency, I authorize the Event organizers and their designated personnel to secure and administer medical treatment and/or transport me to a medical facility as they deem necessary.

Publicity Release: I grant permission to the Event organizers to use my name, likeness, photograph, and/or video for promotional and marketing purposes related to the Event.

I have read this waiver and release form, fully understand its terms, and voluntarily agree to be bound by its terms. I acknowledge that I am giving up substantial legal rights by signing this waiver, and I do so voluntarily.
