Animal Rescue League of Northwest Georgia - Dog Adoption Application
This questionnaire must be completed by anyone interesting in adopting a dog from Animal Rescue League of Northwest Georgia. We at ARL are committed to placing dogs in permanent, responsible homes and, at the same time, matching you with a dog which is suitable for you and your lifestyle. Please answer the following questions completely and with as much information as possible to help us make the best match for you.
Once the application is submitted, we will get back with you as quickly as possible. Please be patient; ARL is run as a volunteer organization and our paid work schedules may prevent us from responding as quickly as we would like.
PLEASE NOTE: Providing false information will result in the forfeiture of both the adoption fee and the adopted animal.
