Therapy Dog Concern or Incident Report Form
Therapy Dogs Australia strives to promote high standards of professionalism and best practices within the Animal Assisted Therapy Industry. 
This form is available to be completed (anonymously if you wish) to report an incident related to a working Therapy Dog which you feel that Therapy Dogs Australia should be made aware of. 
This form can be completed by any person who has either had direct contact with a dog, or has secondhand information that they believe is valid to share. 
Although we are not a regulatory body, we will do our best to look in to all reports to the extent that we have access, to gather further information. Please provide as much information as you can to assist us with creating accountability and professionalism in our industry.
