This is to request a spot in our after school program. Please fill this form out per child you wish to enroll. Walsh Elementary has been added to our route this year, if you do not see your school listed please submitted the form and we will see if the school can be added. Please note that I can not guarantee that new school not listed below can be added.

PAYMENT POLICY: Payments for Club Summit are charged weekly, these charges must be paid in FULL the Thursday prior to the week of Club Summit Pickup. Charges are NOT adjusted for days a student is absent. *

ATTENDANCE POLICY: When your child is going to miss due to illness or personal reasons, the parent must fill out the Daily Changes form BEFORE 2:00 PM Failure to do so will result in a $25.00 NO CALL FEE that will be added to the account in addition to your regular Club Summit Tuition (After the 2nd no call the fee will rise $5.00 each time). Being absent does not qualify for a refund, your Tuition is based on your enrollment that you submit through this form NOT on the days you attend. *
