The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. THE STAGE GLOBAL has put in place preventative measures to reduce the spread of COVID-19; however, THE STAGE GLOBAL cannot guarantee that you or your immediate family will not become infected with COVID-19. Further, attending classes at THE STAGE could increase your risk and your immediate family's risk of contracting COVID-19.

By electronically signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my immediate family and I may be exposed to or infected by COVID-19 by attending classes at THE STAGE GLOBAL and that such exposure or infection may result in personal injury, illness, permanent injury, or death. I understand that the risk of becoming exposed to or infected by COVID-19 at the studio may result from the actions, omissions, or negligence of participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my immediate family or myself (including, but not limited to: personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my attendance at THE STAGE GLOBAL or participation in it's programming ("claims"). I expressly agree and contract, on behalf of myself, my heirs, executors, administrators, successors, and assigns- that THE STAGE GLOBAL and its insurers, employees, officers, directors, and associates shall not be liable for any damages arising from exposure or infection of any kind including but not limited to COVID-19 on or about the premises or as a result of the use of the studio's equipment and facilities; regardless of whether such injuries result in whole or in part, from the negligence of the studio. I hereby release, covenant not to sue, discharge, and hold harmless THE STAGE GLOBAL and its employees, agents, and representatives, of and from the claims- including liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of THE STAGE GLOBAL (its employees, agents, and representatives), whether a COVID-19 infection occurs before, during, or after participation in any studio program. In addition to understanding the risk, I affirm that should I, or anyone withing my household, or anyone I have come into close contact with, develops COVID-19 like symptoms, or tests positive for the virus or antibodies at any point up to 21 days after attending the studio, I will contact THE STAGE GLOBAL immediately - and if I suspect that I have come in contact with COVID-19, or am experiencing symptoms, I WILL NOT attend classes or programs at THE STAGE GLOBAL *
