In consideration for being allowed to participate in the Shawnee Mission South High School Marching Band, Symphonic Band and/or Jazz Band, the undersigned student and his/her parent or legal guardian hereby agree that the uniform that has been or will be checked out to said student will be returned on or before the date specified by the Band Director, in complete and good condition.
Uniforms will be professionally dry cleaned by the Uniform committee.
For purposes of this contract, “complete and good condition,” and replacement value shall be defined as follows:
Band Coat $241.88
PR Gauntlets $39.28
Shako Box $10.50
Issued Hanger $2.00
Garment Bag $17.50
DM Cape $59.95
All band dues must be paid in full and this contract returned in order to obtain your band uniform for the marching band season.
The undersigned further agree that failure to return the uniform in “complete and good condition” shall result in the replacement value of the missing parts of the uniform up to the replacement value of the entire uniform being charged to the student by the Shawnee Mission School District.

Rompin' Stompin' Raider Band Permission Slip
My student has my permission to participate in all activities that involve travel shown in the 2019-2020 Calendar of Events posted on the band’s website. I understand that other events may be added to this schedule and unless I otherwise inform the South Band Director by phone, e-mail, or a note, this permission form will cover those events.
My student DOES NOT have my permission to participate in band activities described above.
Band Uniform and Activity Fee
I will submit a CHECK for $75.00 to "SMS Band" for my student's band uniform and activity fee.
I authorize payment of $75.00 from my student's BAND STUDENT ACCOUNT for my student's band uniform and activity fee.
My student does not need marching shoes this year.
I will submit a CHECK for $30.00 to "SMS Band" for my student's marching shoes. (May be combined in one check with the uniform fee.)
I authorize payment of $30.00 from my student's BAND STUDENT ACCOUNT for my student's marching shoes.
Band Booster Club Membership
I will submit a CHECK for $30.00 to the "SMS Band Boosters" for our family's Band Booster Club membership dues.
I authorize payment of $30.00 from my student's BAND STUDENT ACCOUNT for our family's Band Booster Club membership dues.
I have indicated payment of our family's Band Booster Club membership dues on another student's form.
If your child needs to rent an instrument from the school district, the district's Instrument Rental Contract must be completed and payment of $60 must be submitted to "SM Schools". (Instrument Rental Contract can be printed from the band website
, in the Parent Boosters Forms section.)
