Please complete the following form in its entirety to request supplies for your upcoming trip and one of our officers will be in touch with you.

One request form per trip is allowed for each organization. Organizations are required to complete the Post-Trip Feedback Form after returning from their trip, see the last page of this form for further details. The Post-Trip Feedback Form will also give organizations an opportunity to submit pictures and anecdotes from their trip, which can be used to raise awareness of sustainability and the medical conditions seen during the trip.

*Requests must be made at least four weeks prior to date the supplies are needed for the summer. At least two weeks prior if requesting during the school year starting in September. Exceptions may apply.*
