Key worker parent survey - JANUARY 2021
Dear Parent/Guardian,
I regret to tell you that this evening's announcement has been a complete shock to all schools and we are utterly unprepared to begin provision for key worker families with immediate effect. The school will therefore be closed to all children on TUESDAY 5TH JANUARY.
It is our intention to reopen the school on WEDNESDAY 6TH JANUARY.
In order for us to plan effectively for reopening, we ask parents who are identified as KEY WORKERS to please advise us via this form. Please only complete this form if you are classified as a key worker (e.g. school staff, nursery workers, NHS, Care workers etc) and will need your child to return to school because there is no other adult in the family home to care for your child.
Please note that we will only be able to accommodate children whose parents are deemed as key workers AND who do NOT have another adult at home to care for them.
Please complete one form per child.
Thank you for your assistance in this matter.
Once you have completed this form, we will be in touch with a booking form for sessions for the duration of the lockdown.
