Student Registration Form

Welcome! If you're completing this form that means that your child has signed up to participate in their school's Conservation Crew during the 2025-2026 school year! (Note: some schools may elect to slightly alter the name of the group at their school.)  Conservation Crew is sponsored by Conservation Minnesota. 


Please complete the form to register your student. All information will be aggregated, kept anonymous, and remain private. There are two optional demographic questions; this information is requested by funders to better understand the reach of the program.

(collectively “Conservation Minnesota”) permission to use my/my minor child’s name, image, and likeness (collectively, “image”) in any and all photos and/or videos related to Conservation Minnesota’s print and digital promotional outreach materials without providing any compensation to me or my minor child.

I understand that any use of my/my minor child’s image will be made respectfully and responsibly. If there comes a time that I wish for my/my minor child’s image to no longer be used for promotional purposes and outreach materials, I can request that Conservation Minnesota stop using my/my minor child’s image.
