In order for us to adequately plan for the 2018-2019 school year, we are requesting that you submit the following information. This form is to be completed no later than June 30, 2018 for all currently enrolled students. We will make notification only if we are unable to meet your request. Any form submitted after August 10, 2018 will not be acted upon till September 19, 2018, in the order that it was received.
If your child receives special needs transportation please do not complete this form as the Special Education Department will be contacting you regarding your child's transportation.
RECEIPT OF THIS FORM IS DEPENDENT ON YOUR CHILD RECEIVING A BUS STOP FOR THE 2018-2019 SCHOOL YEAR, IF THIS FORM IS NOT RETURNED YOUR CHILD WILL NOT BE ASSIGNED TO A BUS RUN.
Students requesting alternate transportation requires five (5) consecutive days (AM,PM or both) at the same location.
