Please complete this application and submit it to the Islamic Center of South Bay (ICSB) or the Islamic Center of Hawthorne (ICH) office. Please make sure to read the requirements and rules before signing this vendor form.
I, the undersigned vendor agree to hold ICSB and ICH harmless and indemnify them of any claims, loss or damages arising from my business activity during the Seerah Conference Program on August, 27th 2022. Furthermore, I hold ICSB and ICH not responsible for any loss of merchandise or theft or damages to my merchandise and business during the duration of the event. I certify that I am fully responsible for the sale of my merchandise during the program and that it complies with Islamic laws, as well as local, state and federal laws.
Note: Sellers can choose their spots in advance on a first come, first served basis.

Permit fee is $150 for 2 L-Shape tables with 2 chairs regardless of sales and inventory.

Food: 20% of your sales goes to the Masjid ( have to have license).
