OCS - Middle School Night Out Event

Place and purpose of activity:    Middle School Night Out to be held at OCS 
Date of activity:    Friday, April 14
Event Start Time:   5:30 PM       Event End Time: 8:30 PM 
Event Fee: $10 paid through SQUARE

Drop-Off students to the elementary lobby at 5:30 pm. (All students in the building after 3:30 pm will be sent to OMEGA until the event start time.)  Volunteer Parent chaperoned activities. Taco Dinner, Snacks and Desserts are available. 

Pick up in the elementary lobby at 8:30 PM. A parent must come inside for pickup. Students will not be released to the parking lot. 

Suggested attire: Modest athletic wear / comfortable clothes. Students may get messy. 

            The permission slip and $10.00 fee must be submitted by: WEDNESDAY, April 12

I, the parent or guardian of the above-listed children, give my permission for the child to participate in the activity or field trip described above. Should a medical emergency arise, Oakland  Christian School, or its agent, has my permission to seek and authorize emergency medical care for the child in my absence.
