SDS Hip-hop Showcase - Performer Registration
Please provide us with your full name, and all the classes you intend to perform within the SDS Hip-hop showcase.

*NOTE Please only submit 1 performer per submission.  

All performers need to pay $20 each (please include them on your table form). This includes their entry fee plus, certificate & medallion.

Table bookings and payments MUST be in by 2 weeks prior to the showcase (1st of December, 2019)
