THE FORM BELOW IS DESIGNED TO COMMUNICATE NON-EMERGENCY CONCERNS TO THE SCHOOL COUNSELOR. All students may use this form to refer themselves to Ms. Jackson.  Teachers, parents, and friends/peers may also use this form to refer a student to the school counselor.
 
Information from this form will be kept confidential unless I am mandated to report it by law or we agree that it is necessary to share in order to more effectively address the problem/situation.  

Please be sure to provide a working e-mail address and telephone number because these are the main ways I will be able to communicate with you. Please check your email regularly for my response.
