Education and Support Group Participant Agreement of Confidentiality
Confidentiality, a trust of privacy or secrecy of communication and information, is special in a group setting, and is the shared responsibility of all group members and the facilitator(s). The group facilitator will not disclose group communications or information except as provided by law. Group participants must adhere to the following:
• I will not discuss legal and ethical issues and concerns in the group setting.
• I will not discuss or release information, written or oral, regarding any group member to any person(s).
• I understand that violation of these confidentiality principals could result in my termination as a group member.
ABOHLF adheres to professional, legal and ethical guidelines of confidentiality established by state law. Legal and ethical exceptions to confidentiality include: a clear or present danger to harm yourself or another, knowledge of the abuse or neglect of a minor child or incapacitated adult, responses to a court subpoena or as otherwise required by law.
By my signature below, I indicate that I have read carefully and understand this agreement and that I agree to its terms and conditions.
