1. Fill in the form below to register as a Member of HKUST Alumni Wine Association. Please be reminded to choose your membership type & choose your option of payment by FPS (Faster Payment System) or by bank transfer.
- If you choose to pay by FPS (email address: al_ustawa@connect.ust.hk), please process the payment before completing this registration form.
- If you choose to pay by bank transfer, please send us your bank receipt by email (al_ustawa@connect.ust.hk) after completing this registration form.

2. After receiving your registration, we will verify your eligibility for enrolling our association membership. Please enter all required information.
3. For members with more than one degree honoured from HKUST, please provide us with information of your first degree.
4. To be eligible for our membership, you must be 18 years old or above.  By submitting this enrollment form, you acknowledge and agree to this term.  In addition, there are requirements for various types of Membership and respective fees are listed as below:

5. You will be contacted by email after the verification process of eligibility and payment receipt to confirm your membership status.
6. You will receive the membership card by post. Enjoy our member's benefits at our partners with the card!
7. Information provided by applicants will be kept strictly confidential and will only be used for administration purposes.
