Photographic and Video Parent Permission Form
This permission form is produced by Oxford Rugby Club (ORFC) to comply with the club’s
safeguarding policy on the taking and use of images of players under the age of 18 years. The
club will not take or use any image of a player if permission is not granted. If an image is taken it
will only be used by Oxford Rugby Club celebratory and promotional purposes and will not be
forwarded to any third party.
