Throughout each fiscal quarter of the year, Mod Threads Boutique sets aside a portion of its income to donate to a ministry. If you have a ministry in mind that would be a good candidate, please fill out the form below. We will refer to submitted forms each quarter as we select new ministries to support.

*Filling out this form does not guarantee that the ministry will be selected at any point, or in any certain order, but it does make our team aware of the ministry and gives us the opportunity to review it for consideration.
