Bus/Lunch/After Care/School Clothes Application Form (from April 2020)
Please submit this form to apply for all school services from April.
Please note that unless "ANNUAL" service is signed up, school WILL NOT extend the service automatically. Your service will be CANCELLED unless the form is submitted. Cancellation of the service is available upon request; refund will be made from the following month.
Deadline for sign up is February 16th. Sign up is still available after this date but the service is not guaranteed.
Invoice will be sent at the end of February. Please make payment by the due date which is stated on the invoice.
