Are you reserving a locker for yourself or for part of a group? *
If it's for a group, what group is it?
What is the group exec email?
This is the email I will use to communicate with the group about notifications. It is best not to have an individual student as the point of contact.
I understand that locker space is limited, and I will inform WXGS if the locker is no longer needed. *
This is so others may have access to use lockers.
I understand I will keep the locker clean to the best of my abilities, and that if there is a problem with my locker WXGS will notify me via email. *
I understand that WXGS has a master key to all lockers and will alert me via email before they might need to get into my locker. *
Such as: rotting food left in lockers; abandoned items from a student who graduated; failure to re-submit locker application every year and other students need a locker; etc.
I understand that if I were to get a locker, I will only have that locker for one year from the time it is assigned to me. *
You will receive email notification leading up to the end of your lease, and afterwards you may fill out this form again to be put on a wait-list.
