If picking up transcript, please bring your ID. Only the student will be permitted to pick up their transcript.

1. A signed release of information from the person your are requesting information.
2. Name of School
3. Full legal name used while attending school.
4. Year of graduation or last year of attendance.
5. Full date of birth (Month/Day/Year)
