L88 Complaint Form
Members may file a formal complaint by completing this complaint form and submitting it. You may also submit a complaint in writing and give it to a Local 88 Officer or Staff Representative. The complaint will be reviewed and an investigation conducted to provide a response to the member filing the complaint. The response will be in writing. All notes and documentation taken during the investigation will remain confidential and only a summary as written in the response will be provided to the member.
