Girl Scouts Go Behind The Scenes with The Nutcracker Ballet at Center Stage- Participation Form!
We are thrilled you are going to join us for this exciting event!

LEADERS! Please fill out the form below and submit it and ALL PAYMENTS to Center Stage by October 21st in order for us to order the correct number of patches and supplies and have them arrive in time for the event!

Payments can be made via check, cash or credit card. You can stop by the studio, mail it in, or give us a call at 660.747.6133 to pay.

If you are a PARENT, BEFORE filling this out, please first double check with your leader to see if your troop is registering as a troop or individuals.

Each individual attendee- Parents AND Girl Scouts- will need to ALSO COMPLETE the online waiver prior to participating! It can be found at: https://forms.gle/DnCoayUy9buNuj4N6

Total Number of Patches Needed: (A patch is included in the $9 cost per girl. If an adult would like a patch, they will also need to pay the $9 instead of only the $4 cost.) *
Total amount to be paid to Center Stage: ($9 x number of girls plus $4 x number of adults!) Payment must be received by October 21st! *
