QLT Materials Request Form
This form may be used by those outside of the California State University system wishing to adopt CSU Quality Learning and Teaching (QLT) instruments and/or resources. All CSU QLT materials were created through collaborative efforts in the CSU, based on extensive research into effective teaching and learning practices for blended-online course design and delivery.

Our philosophy is that these types of instructional-professional materials should go into the greater educational community for additional use by those who may benefit. As such, CSU QLT instruments and related materials carry the Creative Commons, Attribution-NonCommercial-ShareAlike (CC BY-NC-SA). This license allows others to remix, tweak, and build upon CSU QLT non-commercially, as long as they credit the California State University, Quality Assurance program (qa.csuprojects.org) and license their creations under the identical CC BY-NC-SA terms.

Our reason for asking is to be aware of where and how QLT is being utilized, as well as hearing new ways institutions are coming up with to apply such materials and related processes to improving blended-online teaching and learning. *

As members of my institution remix, tweak, and build upon CSU QLT, credit will be given to the California State University, Quality Assurance program (qa.csuprojects.org) and any resulting QLT materials from our efforts will carry the same license and terms. *
