How it works:
You will need to fill this form out COMPLETELY and must be submitted at least 48 hours BEFORE THE SCHEDULED workshop if you chose the LIVE option.  The answers on this form will help us to prepare for the workshop by addressing specific topics mentioned in your Intake. 

This Workshop is Live and will include a Q&A so the information on your Intake will also help us to prepare to answer specific questions you might have regarding your evidence.
