RSC Winter Training Registration 2018
Non-member cost will be $100 for 8, 2 hour sessions.
RSC member discount will be $75 for 8, 2 hour sessions.
Each session will consist of foot skills training and futsal games.
Included with your registration will be an RSC T-Shirt.

Who: This is open to our U9 Travel Teams up through High School players
What: Training activities with a regular outdoor ball and with a futsal ball. Small sided games and futsal games.
Where: Training takes place at Craig High School in the field house and the main gymnasium
Why: To stay in shape, have fun, and get many touches on the ball which will ultimately make you a better player

RSC Winter Training Waiver *
By checking the box below, We hereby acknowledge and certify via electronic submission/signature that we are the parents/legal guardians for the player/participant listed herein. We acknowledge and fully understand that the player is engaging in activities that involve risk of injury, including permanent disability or death, and severe social and economic losses. We agree to save and hold harmless, to indemnify and not to sue the Rock Soccer Club (RSC), its directors, officers, employees, coaches, managers, agents, sponsors and associated personnel including those of its affiliated organizations, and the owners and lessors of premises used to conduct the event (hereinafter collectively referred to as releasees) from all liability, loss, cost, claim or damage whatsoever, including death or damage to property, caused, or claimed to be caused, and any injury or accident resulting from participation or alleged to be caused in whole or in part by the negligence of the releasees. This waiver and release applies to any and all activities, including arrival and departure from RSC, public or private property. I/we (above indicated) hereby acknowledge that I/we have reviewed this form and caused it to be electronically executed with the intent to be bound to the terms contained herein.
Member Safety *
Checking the box below indicates that you have read and accept the RSC member safety statement. Safety is the first priority of the Rock Soccer Club. We have established procedures designed to protect all participates in the club. All coaches are licensed by the Wisconsin Youth Soccer Association. This licensing includes bi-annual background checks. All team managers and board members have bi-annual background checks. Additionally our coaches receives specialized training on concussion symptoms and protocols. Bullying and hazing will have zero tolerance within our club. Unsafe horseplay during practice times puts players at additional risk and will be closely controlled by the coaching staff. Discipline guidelines for unsafe behavior can be found under “For-players” on the RSC website
Concussion Safety *
Concussion Agreement Required by Wisconsin Law. I acknowledge that I have read the Wisconsin Concussion Fact Sheet for Parents which is found on the Rock Soccer Website under “For Players -Concussion Information and Player Document Links.” I know what a concussion is and how it may be caused. I also understand the common signs, symptoms, and behaviors. I agree that my child must be removed from practice/play if a concussion is suspected. I understand that it is my responsibility to seek medical treatment if a suspected concussion is reported to me. I understand that my child cannot return to practice/play until providing written clearance from an appropriate health care provider to his/her coach. I understand the possible consequences of my child returning to practice/play too soon. Reference Wisconsin Concussion Law - Act 172 - Statute 118.293.
Sportsmanship *
By checking the box below, we hereby acknowledge our willingness as parents/legal guardians to follow and support our players in following the Code of Conduct found on the RSC Website under For "Players/Rock Soccer Safety Procedures." The Rock Soccer Club is a member of the The Madison Area Youth Soccer Association (MAYSA). We use an abide by the MAYSA Code of Ethics & Conduct is intended to help all stakeholders become the best they can be. MAYSA stakeholders include players, teams, parents, coaches, clubs and administrators. All are challenged to not only meet a high standard of sportsmanship, Fair Play and respect for the game but to set a high standard regardless of the actions or behavior of others. All stakeholders representing the Rock Soccer Club will display professional and positive behaviors, both on the field, sidelines, and spectator areas. All participants are encouraged to be familiar with the FIFA Laws of the Game and any US Youth Soccer, Wisconsin Youth Soccer Association or MAYSA/Stateline adaptations. In addition, all participants are expected to show the utmost respect to referees at all times and abide by MAYSA’s No Tolerance Policy for Referee Abuse (see Section 8 of Policies & Procedures). Participants need not agree with the decisions of referees but are fully expected to accept and abide by the decisions. No individual may confront a game official at halftime, during the game or post game. The complete code of conduct is available on the website.
Use of Photographs *
By checking the box below, we hereby acknowledge that the Rock Soccer Club may occasionally use images of my child in photographs in social media, website, or other marketing methods. Please note, in today's digital world, it is very difficult for the club to control all images of any player. Parents, team managers, coaches, and other players often share the fun and success of teams via photographs on Facebook, posts to the Rock Soccer website, or other means. We do recognize that there are certain instances where the face of a player needs to be protected. If you decline this wavier, we ask that you send an email a photograph to the club and we will have a volunteer attempt to screen images during the season.
