Permission to Use Photographs and Videos: 

I give Hoʻaka Mana/Kukulu ʻIke and their agents my permission to use photographs and/or videos of me for marketing and grant funding purposes.  I grant them the right to use the photograph and/or videos to illustrate program activities in educational, instructional, and publicity materials, including meetings, presentations, publications, websites, newsletters, advertising and social media. I hereby release and discharge Hoʻaka Mana/Kukulu ʻIke  and their agents from any and all claims, demands, and causes of action that I have or may have by reason of this authorization or use of my photograph or video.
