-Bu formda adı geçen herkesin nüfus kağıdı fotokopisinin (renkli) forma ek olarak sunulması gerekmektedir. Telefon, faks, e-mail vb yollarla beyan kabul edilmeyecektir. Bu yazılı beyan dışında hiçbir şekilde öğrenci başka kişilere teslim edilmeyecektir. -Formun eksiksiz, karalamasız ve mavi tükenmez kalemle doldurulması gerekmektedir. / The form must be filled out completely, without any scribbles, and with a blue ballpoint pen.

-Form doldurmayan velilerimizin çocukları anne ve babaları dışında kimseye teslim edilmeyecektir. / The children of parents who do not fill out the form will not be handed over to anyone other than their mother and father.

-Bu form okul saatleri içerisinde geçerlidir. Öğrencilerin okula geliş ve gidişlerinden veli sorumludur. / This form is valid during school hours. Parents are responsible for their children's arrival and departure from school.

-Formun veli tarafından elden okul müdürlüğümüze teslim edilmesi gerekmektedir. / The form must be delivered to our school administration by the parent in person.
