  • Collect your order information so we can process your reimbursement.

  • Understand if any customers experienced unexpected effects to assist with our internal review.

  • Ensure all recalled product is properly returned, tested, and disposed of.

  • We are only collecting your order number to match your return and reimbursement. No personal names or email addresses are required in this form.

  • Your responses will be used solely for recall and safety purposes and will not effect reimbursement.

  • Once you submit the form, we will email a prepaid return label, if one has not already been sent to you. Reimbursement (refund, store credit, or replacement product) will be processed after the return is received.
