Leave fields blank for any info you do not want to include. The information provided below will only be used in the Hyer school directory.

Please note the directory information will be made available electronically via online website (www.DirectorySpot.net) and mobile device app (Directory Spot) to all parents who purchase access.

The Hyer PTA will include email addresses for consenting family members. By consenting, you authorize the Hyer PTA to use the email address(es) you provide below solely for use in the Hyer school directory, and you acknowledge that the email address(es) will be available to other parents at your school who purchase the Hyer school directory. If you would like to have your email address and/or that of your spouse or student listed in the Hyer school directory, please complete the information in the fields below.
