Team Meetings 2018-19 (Tier 1)
Instructions:
A student is discussed for 10 minutes using this protocol. In the instance of a Team Meeting, the team should choose three students of highest priority, and submit a form for each. Every time a student is discussed, a team member will complete (a new version of) this form. Guidance Counselors facilitate the discussion about each student with three questions:
What data supports our concerns and describes this student’s challenge?
What interventions will we engage to support this student?
What is our desired outcome, and when will we agree to follow up and determine next steps?
Please use non-evaluative and fact-based language in describing student performance in qualitative and quantitative terms, whether social and learning behaviors or performance on assessments.
