ENROLLMENT QUESTIONNAIRE
Welcome to McFarland Unified School District!  

This is the first step in registering students new to McFarland Unified School District and this survey allows you to quickly start the process of registering your student(s) for school. A helpline is listed for assistance at the bottom of this information if you have any questions or need assistance obtaining required documents.  

Parents may choose to upload pictures or copies of the following required documents to complete their online registration in Step 2. 

For 2024-2025 TK or Kinder registration you will need the following:
   
  1. Proof of age - birth certificate, baptismal certificate, passport, or when none of the above documents is obtainable, an affidavit of the parent/guardian)  
  2. Up-to-date Immunization Card; with TB results March 1, 2024 or after (if available)
  3. Physical Exam dated March 1, 2024 or after    *WAIVERS ARE AVAILABLE*
  4. Dental Exam dated March 1, 2024 or after        *WAIVERS ARE AVAILABLE*
  5. Proof of current address under parent/guardian name - property tax payment receipt, rental property contract, lease, or payment receipt, utility service contract, statement, or payment receipt, pay stub, voter registration, correspondence from a government agency, or a declaration of residency executed by the student's parent/guardian.  If the student is an unaccompanied youth as defined in 42 USC 11434a, a declaration of residency executed by the student, or, if the student is residing in the home of a caregiving adult within district boundaries, an affidavit executed by the caregiving adult in accordance with Family Code 6552)
  6. Emergency contact names, phone numbers, and addresses
  7. CALWORKS 7-digit ID# (if applicable)

2024-2025 Eligibility birth dates for TK or Kindergarten:
Kindergarten: If your child was born on or before September 1, 2019  
Transitional Kindergarten: Children born between September 2, 2019 and June 2, 2020

To start the registration process, please complete the following survey. You will need to complete one survey for each student you are registering. A representative from our Child Welfare and Attendance office will contact you regarding next steps.

-Immediate enrollment in the school they last attended (school of origin) or the local school where you are currently staying, even if you do not have all the documents normally required at the time of enrollment
-Continue to attend their school of origin if requester by you and it is in their best interest
-Receive transportation to and from their school of origin, the same special programs and services, if needed, as provided to all the other children including free meals and Title I
-Receive full protections and services provided under all federal and state laws, as it relates to homeless children, youth, and their families
