Are you aware that a new environment is stressful for your pet, and they may exhibit uncharacteristic behavior? Are you familiar with the 3-3-3 rule and will let the pet decompress for a couple of days before introducing to all animals?

 Has anyone in the household ever been accused or convicted of animal cruelty, neglect, or abandonment?

Please confirm that you understand that the dog belongs to the rescue and you will inform the rescue of any issues, including medical. Rescue will make an appointment at a vet approved by the rescue per foster’s request if needed and will pay directly to a vet only. If a foster takes a dog to a vet at his own discretion without previous approval of a vet and services to be provided, he/she will be responsible for paying the bill.
